Please contact your Teacher or School to obtain your Student Registration Code. They'll provide a registration slip similar to the picture below (the Student Registration Code will be unique for each student).
In consideration of the Program in which my child is participating and/or the prizes which have been awarded to my child by the Success in Education Foundation, The Road to Success, the Road to Success Sponsors, the school district, and or this Elementary School (collectively, the "Sponsors"), on behalf of myself and my child, I hereby waive, release, discharge and hold harmless each of the Sponsors; their parent companies, distributors, subsidiaries, affiliates, advertising, promotion, production, and fulfillment agencies and their respective officers, directors, employees, representatives, agents, successors and assigns from and against, without limitation, any losses, damages, claims, demands, actions, rights, costs and expenses, including reasonable attorney's fees, that my child or our heirs, successors and assigns or any third party may have arising out of my or their use or misuse of any prize and/or a contest in order to receive a prize including but not limited to personal injuries, death and disability.
I understand that my child may be photographed and/or recorded by the Sponsors and by various media (e.g., newspaper, radio and television) during Road to Success - school events. I consent to such activities on behalf of my child so long as all photographs and recorded media are used solely for promotional purposes associated with the Road to Success program.
I represent and warrant that I understand the risks, both ordinary and extraordinary, associated with the prize and/or contest and agree to hold the Sponsors harmless while my child is participating in this contest and/or using any prize awarded to my child including but not limited to travel involved with the prize.
I acknowledge that the Sponsors have not made any warranties with respect to any prize. No substitution will be allowed for any prize. Prizes are not transferable and not redeemable for cash. I also acknowledge that neither the Sponsors nor their agents have arranged for or carry and insurance of any kind for my benefit or that of my child or our heirs, executors or administrators, relative to my child's use of any prize; and that I am solely responsible for obtaining and paying for any life, travel, accident, property or other insurance relative to my child's use of any prize and any travel involved with the prize. I agree to return to the Sponsors any prize that may be awarded to my child if any statement made by me herein is false, or if I do not abide by the Official Rules and any terms or conditions prescribed by the Sponsors.
